BioNTech Halts mRNA Colorectal Cancer Vaccine Trial

An independent review found that BioNTech’s experimental personalized mRNA treatment was unlikely to improve survival in certain high-risk colorectal cancer patients, marking a setback for the company’s cancer-vaccine program.

BioNTech has decided to terminate a Phase 2 clinical trial testing its personalized mRNA cancer immunotherapy, autogene cevumeran, in patients with high-risk colorectal cancer. The decision followed a review by an independent Data Safety Monitoring Board, which concluded that the treatment was unlikely to demonstrate the intended survival benefit.

The trial involved patients with Stage II high-risk or Stage III colorectal cancer who had undergone surgery but still had detectable circulating tumour DNA, an indicator that cancer cells may remain in the body. The treatment was designed to train patients’ immune systems to recognize tumour-specific mutations and attack cancer cells.

The setback is particularly notable because enthusiasm surrounding mRNA cancer vaccines has increased sharply following encouraging results from Moderna and Merck in melanoma. However, colorectal cancer can present a more difficult target for immunotherapy, particularly when tumours have biological characteristics that make them less responsive to immune-based treatments.

The independent review found not only a lack of sufficient evidence that the treatment could improve survival, but also a numerical imbalance in survival outcomes between the treatment and control groups. BioNTech’s U.S.-listed shares fell sharply following the announcement, reflecting investor concerns about the setback.

Despite the failure of this particular colorectal-cancer study, BioNTech has not abandoned mRNA-based cancer research. The company continues to evaluate autogene cevumeran in pancreatic cancer, while also expanding its oncology pipeline into other approaches such as antibody-drug conjugates and bispecific therapies. The result therefore represents a setback for one program rather than evidence that mRNA cancer vaccines as a whole cannot work.
